Supporting self-management of chronic illness

This course is for those working in various care and support roles in the community services industry who wish to develop skills in helping people with their self-management of chronic disease.

This skill set has been endorsed by the industry as appropriate for people who hold a qualification at Certificate III level or higher in an area involving the provision of direct client care or support.

Offered as a Short Course minimum numbers required.

Outline

  • CHCCCS001 Address the needs of people with chronic disease

  • CHCCCS023 Support independence and wellbeing.

Entry Requirements

This skill set is suitable for individuals who hold a qualification at Certificate III level or higher in an area involving provision of direct client care or support.

Delivery

  • Face-to-face in a small group training environment.

  • The unit ‘Support Independence and Well-being’ requires 120 hours of work placement involving direct support work.

  • Additionally, the participant must be able to demonstrate and safely support at least three people to enhance independence and well-being.

Pathways Information

These units provide credit towards a range of qualifications in the CHC Community Services Training Package or HLT Health Training Package at Certificate III or IV level.

Duration

The course duration is dependent upon previous skills and experience in the industry area together with applicable credit transfer if this should apply.

Full course fee $750
